Complete the Form: Take your time to carefully fill out the application form, providing all the necessary information. Be sure to read and follow the instructions on the form to ensure a smooth and efficient evaluation of your request.

Prepare the Application Fee: Include a $25 non-refundable application fee with your submission. We accept checks or money orders, made payable to Texas Pulmonary Foundation. Please do not send cash.

Mail Your Application: Once you have completed the form and prepared the application fee, place them together in an envelope and mail it to the following address:

5. Confirmation and Review: Upon receiving your application, we will send you a confirmation email. Our team will then carefully review your request for financial assistance and notify you of the outcome. Please allow 5-7 business days for the review process.
