We understand that seeking financial assistance can be crucial to achieving your goals, and we are here to help. Our foundation is committed to supporting those in need, and we have made the application process as simple and transparent as possible. Please follow the steps below to apply for financial assistance from our foundation.
 

  1. Download the Application Form: Click above link to download the PDF form. 
     

  2. Complete the Form: Take your time to carefully fill out the application form, providing all the necessary information. Be sure to read and follow the instructions on the form to ensure a smooth and efficient evaluation of your request.
     

  3. Prepare the Application Fee: Include a $25 non-refundable application fee with your submission. We accept checks or money orders, made payable to Texas Pulmonary Foundation. Please do not send cash.
     

  4. Mail Your Application: Once you have completed the form and prepared the application fee, place them together in an envelope and mail it to the following address:

  5. Confirmation and Review: Upon receiving your application, we will send you a confirmation email. Our team will then carefully review your request for financial assistance and notify you of the outcome. Please allow 5-7 business days for the review process.
